First Diagnosis Dread Disease Benefit Rider
This rider provides the First Diagnosis Dread Disease Benefit amount shown on the policy schedule. The first diagnosis of a covered dread
disease must be after the waiting period and while the rider is in force with respect to the insured. Each insured is limited to one First
Diagnosis Dread Disease Benefit amount under the terms of this rider.
The Dread diseases are any of the diseases listed below which are first manifested after the effective date and after a waiting period shown
on the policy schedule. The disease must be diagnosed by a physician based on generally accepted diagnostic criteria. The covered
diseases are:

First Diagnosis Heart Attack & First Major Heart Surgery Benefit Rider
This rider is designed to provide coverage if you receive a first diagnosis heart attack or if your first major heart surgery is performed. UTA
will pay the insured the first diagnosis Heart attack benefit, reduced by the first major heart surgery benefit if previously paid for the insured
under this rider.The first Major heart surgery benefit is equal to twenty percent (20%) of the First Diagnosis heart attack benefit. Both must
be after the waiting period and this rider is in force with respect to the insured. Each insured is limited to one (1) First Diagnosis Heart
Attack Benefit and First Major Heart Surgery Benefit under the terms of this rider.
Cash Value Benefit Rider
This Rider will pay You a Cash Value benefit beginning with the day after the Fifth Rider Anniversary date. The cash value benefit paid will
be the Original Premium paid on or after the rider Effective date less any claims paid multiplied by the applicable percentage as shown in
the rider.
The Cash Value Benefit will be paid if any of the following conditions are met:

You surrender the policy after the Fifth Rider Anniversary Date 
o
You die after the Fifth Rider Anniversary date 
o
You fail to pay the premium required to keep this Rider in force after the Fifth Rider Anniversary Date 
o
the Policy to which this Rider is attached terminates after the Fifth Rider Anniversary Date
